QUESTIONNAIRE MANUAL

The purpose of this program is to create HTML forms to be embedded in a HTML page.

A questionnaire is used to collect data for different purposes; from marketing data to guest services etc. It acts like a HTML Form

For this purpose we need categories, sub categories and questions. Organizing the categories, sub categories and questions results in a questionnaire.
For different purposes there can be different questionnaires.

The questionnaires like Categories, Sub Categories and Questions can have icons. The icon cannot be larger than 100 * 100 pixels, 40KB and can be .jpg or .png.

Each questionnaire represents 1 html form and has variables related to background color, font, font color, font size.
With regards to back ground picture: The form can be quite long and it is not allowed to have  a background picture within the form, as the form will be embedded anyway.

To Add

1. Enter the Questionnaire name.
2. Set the Background Color, Font , Font size, Font Color.
3. You can also upload an icon applicable to the questionnaire.
4. Click "submit" when done.

To Edit

1. Click the "edit" icon and start editing the data.
2. Click "submit" when done.
3. Click "cancel" if you don't want to make any changes.

To Delete (deactivate)

1. Select the questionnaire name that you want to deactivate, click the "deactivate" button.
2. Are you sure you want to de-activate this record?
3. Click "OK".

To Reactivate

1. UN-tick the box “hide inactive” to show the in-active records.
2. Navigate to the questionnaire name that you want to re-activate, click "re-activate" button.
3. Are you sure you want to re-activate this record?
4. Click "OK".
